Assisted Home Care Services offers skilled and non-skilled care services to seniors and other adults at home, in the hospital and in senior living homes and facilities.

Located in Brooklyn and The Bronx, New York, Assisted Home Care Services provides home health care and nursing as well as non-medical home care, companion care and homemaking services. Options we provide include 24-hour live-in care and home care by the hour.

Our clients include seniors as well as adults of all ages recovering from surgery, illness or injuries.

In general, our home care services can be grouped into four main categories:

Certified Home Health Aides (HHA) & Personal Care Aides (PCA)

Our certified home health aides are kind, compassionate, and experienced caregivers who speak your language and understand your culture. We work diligently with our patients and their families to identify the caregivers that will best fit their individual needs.


Registered nurses ensure we provide the best quality home care to our patients. By working closely with our patients and providing additional training to our aides, we strive to care for our patients as we would for our own family members.

Managed Long Term Care (MLTC)

Medicaid patients qualify for home care services if they are enrolled in Managed Long Term Care Plans. Assisted Home Care Services understands the difficulties and intricacies of qualifying and we take a personal approach with every patient to ensure efficient and painless enrollment process.

Traumatic Brain Injury

This program is designed to make it possible for individuals who have suffered a TBI to stay in their homes and communities. Home and Community Based Services TBI waiver services are designed to address the unique needs of eligible individuals and promote a service delivery system to assure maximum participant choice.  When waiver services are appropriately combined with other services, it enables individuals with TBI to successfully live in their communities.

Nursing Home Transition and Diversion (NHTD)

The NHTD waiver program is a home and community-based program that helps New York´s Medicaid-eligible seniors and people with physical disabilities receive comprehensive services they need while they live in a community-based setting, rather than in a nursing home, congregate care setting, or other institution. Medical Services

Medication management, administration of injections, wound care and monitoring of health status


Program to enable individuals who have suffered a TBI to stay in their homes and communities.

Alzheimer's Care

Provide fun and stimulate joy, reduce unwanted behaviors, allow client to succeed with challenges.

Hygiene Assistance

Hygiene supervision for the client which may include assistance with bathing, toileting and grooming.

Light Housekeeping

Cleaning, dusting, vacuuming or mopping client’s kitchen, bathroom, bedroom and main living areas.

Stimulation Activities

Going on walks with client and encouraging participation in stimulating activities.

Medication Reminders

Ensuring that clients are taking their prescribed meds in the right amounts and at the right time.

Laundry & Linens

Washing of client’s personal laundry, washing the bed linens and regularly changing the bed.

Ambulation Assistance

Supervising the client with walking, getting in and out of chairs and bed and helping to prevent falls.

Meal Preparation

Planning, preparing and serving nutritious meals and snacks according to dietary requirements.

Shopping & Errands

Personal shopping for clients, performing domestic and personal related errands outside the home.


Driving and or escorting client to their doctor or other appointments as well as social events.
